RiseWell Community Services is seeking a dedicated Bilingual (Spanish speaking) Care Coordinator to join our Care Coordination Department. This remote role requires field visits across Long Island (Nassau & Suffolk County).

Salary: 38k-40k Annually
In this role, you will provide care coordination and health promotion to individuals with severe mental illness and chronic health conditions to avoid costly inpatient hospitalizations, emergency room visits and unnecessary care. Oversees and coordinates access to all services (behavioral, medical health and social service supports) to assist individuals to stay healthy. Completes all required documentation according to standards and no later than 48 hours of service delivery.
QUALIFICATIONS:
Minimum of a Bachelor’s degree or a CASAC with at least 2 years of experience serving people with mental illness, developmental disabilities or alcoholism or substance abuse.Clean, valid NYS Driver’s License. Fingerprinting, criminal record check, approval from NYS Office of Mental Health.
