RiseWell is currently seeking experienced candidates for the Intake Coordinator position within our clinical department at our Patchogue location.
JOB TITLE: Intake Coordinator
DEPARTMENT: Clinical Services
Pay: $45,000.00 per year
JOB FUNCTION:
Responsible for screening and conducting intakes for individuals seeking admission to the PROS Program. Conducts assessments to determine an individual’s needs and preferences and develops recommendations for the person’s Individual Recovery Plan. Conducts new member orientation and coordinates with all referral sources and collateral. Maintains appropriate records according to program regulations.
