RiseWell Community Services is looking for a full-time/part-time educator for the Community Oriented Recovery & Empowerment Program.
QUALIFICATIONS:
Minimum High School Diploma, Peer Certification, and experience serving people with mental illness, developmental disabilities or alcoholism, or substance use. A clean, valid NYS Driver’s License is preferred. Fingerprinting, criminal record check, approval from NYS Office of Mental Health.
CORE & ESSENTIAL RESPONSIBILITY:
Provide services to Medicaid beneficiaries with mental illness and/or substance use disorders to receive services in their own home or community. Help members prevent and manage chronic health conditions and recovery from serious mental illness and substance use disorder. Provide services that support the acquisition of living, employment, and social skills and be offered in home and community-based settings that promote hope and encourage individuals. Link and accompany individuals to needed social services and community resources as necessary to facilitate their recovery goals.
SALARY: $17.50 Hourly
