RiseWell Community Services is seeking a full-time Program Manager (LCSW) to join the Home Best Program. In this role, you will be responsible for the overall operation of the Home Based Empowerment Support Team (Home-BEST) Program.

Location: Remote (Staff are expected to report to West Babylon for meetings & supervisions)
Hours: 35 Hours per week
Salary: $65k Annually
Qualifications:
LCSW as credentialed by New York State with a minimum of 3 years of experience serving children and adults with serious mental illness, developmental disabilities or substance abuse in Nassau County Only. Must be proficient in conducting appropriate screenings and either performing or arranging for more detailed assessments when needed (e.g., high-risk substance use or mental health related indicators, harm to self/others, abuse/neglect and domestic violence). Clean, valid NYS Driver’s License, Fingerprinting, criminal record check and approval from NYS Office of Mental Health.
Job Function:
Responsible for the overall operation of the Home Based Empowerment Support Team (Home-BEST) Program. Provides staff training, supervision, evaluation and development. Provides oversight of documentation and processes to ensure compliance with policies and procedures, regulation and guidelines. Provides guidance and direction to all staff and interfaces with community organizations and interfacing departments. Responsible for ensuring the team’s delivery to provide families with adequate support and access to resources that encourage development of skills necessary to improve quality of life and outcomes for high-need individuals with SMI. Reports to the Medford Site, as needed for meetings and supervision.
