RiseWell Community Services is looking for a full-time Therapist to join the Clinical Department’s Suffolk County Childhood Trauma & Service Center.
In this position, you will responsible for providing clinical evidence-based services with a focus on integrated mental health, physical health, and substance use recovery. Services will be provided by a multi-disciplinary treatment team with an overall goal to improve consumer health outcomes. Service delivery will occur on-site, in vivo, and in the community. A caseload of children and families will be maintained while providing individual, group, and family sessions.

Pay: $60,000.00 per year
QUALIFICATIONS:
Valid NYS LMSW or LMHC with minimum 1 year of experience treating traumatic stress among youth and families. Valid and clean NY State driver license, fingerprinting, criminal record check, and approval from NYS required.
CORE & ESSENTIAL RESPONSIBILITY:
A. Work as part of a multidisciplinary team in order to address all components of care for clients enrolled in SCCTS services
B. Maintain a caseload of clients to provide therapeutic services in varied modalities.
a. Responsible for completion of all documentation for assigned caseload within 48 hrs.
b. Follow lost to contact procedure for clients and document all outreach in progress notes.
C. Complete initial evaluations and all required intake documentation.
a. Include preliminary diagnoses.
b. Source of the referral
c. Reason for seeking care, as stated by the consumer or other individuals who are significantly involved.
d. Identification of consumer’s immediate clinical care needs related to the diagnosis of for mental and substance use disorders.
e. Include list of current prescriptions.
f. Complete Risk Assessment.
g. Complete Violence and Suicide Risk Assessment.
h. Complete Comprehensive Needs Assessment.
i. Determination of military status.
D. Provide group therapy services to clients.
a. Develop and facilitate groups that will meet the needs of the current population.
b. Complete group notes within 48 hours of service.
E. Provide services both on site and in the community at various locations.
F. Attend supervision, team meetings, conference calls, and trainings as required to aid in the integrative process of creating a patient centered, strength based treatment plan.
G. Initiate and maintain positive relationships with clients, families, co-workers and community providers.
H. Provide client with linkages to community resources when necessary in order to maintain a continuum of care
I. Provide recovery oriented services to individuals in a person centered manner in accordance with SAMSHA’s guiding principles of recovery.
a. Assists in the maintenance of the clinic and ensure the environment promotes recovery.
J. Provide crisis services and response.
a. Participate in rotating on call schedule.
b. Support the ability of an individual in crisis to address stressors without requiring hospitalization when possible.
c. Maintain patient confidentiality and comply with all HIPAA standards.
K. Actively collaborate with internal and external providers to coordinate care and implement interventions in accordance with protocols, standards of care and current research.
L. Display commitment to the mission of RiseWell and its values.
M. Notify Program Supervisor of any significant events that may occur, as per Agency Incident Reporting Guidelines.
N. Education and Training.
a. Have a working knowledge and understanding of evidenced–based practices, including harm reduction and motivational interviewing.
b. Complete all necessary LMS, CPI and other assigned trainings.
c. Attend outside trainings to further develop area of expertise.
d. Task supervision of interns and obtaining SIFI certification, if eligible.
e. Pursue advancement of licensing, if eligible.
O. Comply with Agency Compliance Plan and False Claims Policy by preventing, detecting, and reporting any potential healthcare abuse and fraud. Report any potential non-compliance and comply with all other federal, state and local regulations applicable to this job description.
P. Perform all other job duties as assigned.
