Mental Health Treatment
From hospital-referred intensive care to independent therapy and beyond, RiseWell tailors mental health services to an individual’s needs. We offer a wide array of services through our intensive outpatient programs including individual and group therapy, medication management, substance abuse recovery services, health screening, remote telehealth as well as in-home treatment. RiseWell Community Services Clinical Services Department serves over 1,400 individuals at a time through Personalized Recovery Oriented Services (PROS) Programs, Outpatient Mental Health and Substance Use Clinics, and Assertive Community Treatment (ACT) Programs.
At RiseWell Community Services’ Wyandanch Clinic, we’re dedicated to providing compassionate, evidence-based care services to individuals, groups, and families facing mental health challenges of all types, as well as co-occurring substance use disorders, including Medication Assisted Treatment (MAT).

RiseWell is transforming mental health care with our specialized teams in Suffolk County and Queens, NY. Our ACT (Assertive Community Treatment) and FACT (Forensic Assertive Community Treatment) teams provide a full-suite of treatment to individuals dealing with serious mental illness, so they can get the support they truly need.
RiseWell’s ACT services aim to prevent relapse, teach valuable life skills, provide assistance, and connect individuals with essential community resources so they can move forward and thrive in work, school, home life, and social relationships.
Our FACT mobile teams serve individuals diagnosed with severe mental illness whose needs have not yet been met by traditional approaches, and who have a history in the criminal justice system. We support individuals through substance abuse issues, medication and treatment compliance challenges, those under AOT (Assisted Outpatient Treatment under civil court), individuals lacking support networks, those with histories of homelessness, and those with educational or vocational needs. RiseWell is here to help reduce criminal behaviors and prevent recurrent involvement.
Our highly personalized approach is multifaceted, featuring a mobile team that includes a psychiatrist, psychiatric nurse practitioner, registered nurse, social worker, employment specialist, substance abuse specialist, and peer specialist. We believe in quality, convenient care, whether that means meeting individuals in their homes, out in the community, or a location of their choice.
At Recovery Concepts, our two P.R.O.S programs provide personalized attention, wellness classes, and intensive support to help individuals with a mental health diagnosis avoid relapses in both mental health and substance abuse recovery.
Services are offered both on-site and in the community, tailoring programs to assist members in achieving their unique personal goals (for example, employment and education). Throughout our program offerings and workshops, we encourage the active involvement of family, friends, and loved ones in each participant’s treatment journey. Support is essential in recovery!
Our dedicated P.R.O.S staff collaborate with the participants to create an Individualized Recovery Plan (IRP) based on their strengths, personal goals, hopes, and needs. With these in mind, participants have the freedom to choose the services that align best for them!
The services provided include:
Community Rehabilitation and Support (CRS)
Focuses on strengths in order to manage mental and physical health, develop needed skills, and connect with resources to thrive in the community.
Ongoing Rehabilitation and Support (ORS)
Assists individuals in maintaining employment, including employment counseling, advocacy, and support while on the job (at or outside of the workplace!).
Intensive Rehabilitation (IR)
Helps individuals reach specific life goals, such as returning to school, obtaining employment, finding a new place to live, as well as effectively managing their symptoms. This service also provides interventions to reduce the risk of hospitalizations, relapse, loss of housing, or involvement in the criminal justice system.
Clinical Services
Helps each person manage their mental health condition through services including clinical counseling, meditation management, health assessments, symptom monitoring, and treatment for substance abuse.
Integrated Dual Disorder Treatment (IDDT)
Addresses the mental health and substance abuse needs of a person with co-occurring disorders through cognitive-behavioral and harm-reduction approaches.
