Tracy Falkner, LMSW

Tracy Falkner, LMSW, CPRP serves as Chief of Communications & Development at RiseWell Community Services, where she leads the agency’s strategic communications, brand development, and fundraising efforts. In this role, she is focused on elevating RiseWell’s visibility while strengthening community and stakeholder engagement and advancing the organization’s continued growth and impact.

With more than three decades at RiseWell, Ms. Falkner brings a deep understanding of the organization’s programs and the communities it serves. Her leadership experience spans outreach and homeless services, residential and care coordination programs, employment and vocational services, peer support, and Personalized Recovery Oriented Services (PROS), as well as senior volunteer, intergenerational, and family respite programs.

Prior to her current role, Ms. Falkner served as Chief Operating Officer and Deputy Chief Operating Officer, where she oversaw program operations and helped drive agency-wide strategy and service expansion.

Ms. Falkner is a Licensed Master Social Worker and holds a Master of Social Work from Fordham University, with a focus on Social Work Administration.
