Tracy Falkner, LMSW

Tracy Falkner is the Chief Operating Officer of RiseWell Community Services. In her role, Ms. Falkner is focused on continuing to develop RiseWell’s breadth of services across Long Island and NYC, while overseeing program management and serving as a leader of agency strategy. Before becoming Chief Operating Officer, Ms. Falkner served as the Deputy Chief Operating Officer, and prior, as the Senior Director of Outreach Services where she was instrumental in managing and developing outreach and homeless services, residential and care coordination services, employment and vocational programs, peer support as well as Personalized Recovery Oriented Services (PROS). Additionally, she provided oversight of RiseWell Community Services’ senior volunteer and intergenerational programs as well as family respite services. Ms. Falkner has been with RiseWell for over 30 years and is a Licensed Master Social Worker. She received her MSW from Fordham University with an emphasis on Social Work Administration.
