RiseWell’s Clinical Services Department serves over 1,400 individuals at a time through Personalized Recovery Oriented Services (PROS) Programs, Outpatient Mental Health and Substance Use Clinics, and Assertive Community Treatment (ACT) Programs across Queens Kings and Suffolk Counties.
RiseWell Community Services offers over 800 permanent, safe and affordable residential placements in our Supportive Scattered-Site Housing Program. In this program, individuals with disabilities live in apartments or houses throughout Nassau, Suffolk, Queens, Brooklyn and the Bronx.
RiseWell Community Services provides care coordination services in Queens, Kings, Suffolk and Nassau Counties. Individuals diagnosed with serious mental illness, chronic medical conditions, and/or substance abuse disorders are eligible to receive a care coordinator who will help that individual manage all aspects of their healthcare.
RiseWell Community Services provides Care Coordination services to children and youth on Long Island and in New York City. Care Coordination provides family-and-youth driven services which foster health promotion.
