RiseWell Community Services offers over 800 permanent, safe and affordable residential placements in our Supportive Scattered-Site Housing Program. In this program, individuals with disabilities live in apartments or houses throughout Nassau, Suffolk, Queens, Brooklyn and the Bronx.

With over 800 permanent, secure, and affordable housing options, RiseWell Community Services understands that housing IS health. Through our Supportive Scattered-Site Housing Program we seek to transform the way individuals with disabilities experience ‘home’ through nurturing aspirations and enabling them to thrive in their own space. 

In our program, we believe in growth through choice and independence. Our offered supportive scattered-site housing placements specifically targets vulnerable populations, including veterans, individuals who have experienced homelessness, and those transitioning out of adult homes, state psychiatric centers, prisons, and local hospitals. We provide eligible participants the opportunity to live in their own apartments or houses across Suffolk County, Nassau County, Brooklyn, and the Bronx. 

All licensed by the New York State Office of Mental Health, our residential services also encompass programs such as Community Residences, Intensive Support Apartment Programs, and Community Residences/Single Room Occupancies (CR/SRO).

Individuals can initiate a housing application in Nassau and Suffolk Counties via completion of a Single Point of Access (SPA) application found at http://www.spahousingli.org/doc/Application.pdf

Individuals in any borough of NYC can initiate the application process via completion of an HRA-2010e application with a service provider who has access to the web-based application site.

Two community residences in Nassau County which offer on-site staffing, 24-hours a day, 7-days a week. Services include medication monitoring, symptom management, socialization, transportation training, connections to psychiatric and medical services and more.

With two locations located in Queens Village and Brentwood, we offer transitional residential living with 24-hour on-site support staff. Services include medication monitoring, symptom management, socialization, transportation training, connections to psychiatric and medical services, and more. 

Our Brentwood location, Irving Berkowitz Residence, has a temporary housing program specific for individuals from acute hospitals to assist with integration back into the community. 



Participants reside with one or two housemates in the community, with visits from housing counselors several times a week. Services include medication monitoring, symptom management, socialization, transportation training, connections to psychiatric and medical services, and more.

The goal of Mobile Residential Support is to ensure that that participants can maintain their housing, further develop community and daily living skills, and increase their independence. The MRS team provides intensive support services to individuals who reside in Nassau County Scattered Site Supportive Housing within the SPA Housing system

Services include:

  • Medication management and education
  • Community reintegration (including travel training)
  • Daily living skill development
  • Assistance with medical and behavioral health appointments
  • Coordination with participant’s treatment and support team
  • Budgeting and entitlements training
  • Counseling services
  • Substance abuse services and support
  • Crisis intervention (including access to 24 hour emergency line and after-hours warm line)

RiseWell Community Services’ Respite Housing Program offers quality short-term housing for those with mental illness in need. RiseWell is here to assist those seeking a safe haven whether from a challenging living condition, needing a break from surroundings, or facing a dilemma after discharge.

With two convenient Respite Housing locations in Suffolk County, RiseWell is committed to providing a safe and comfortable temporary home for up to 14 days. Must be 18+ years or older with a mental illness.

Herkimer Gardens is a 121-unit mixed-use development with a significant emphasis on Supportive Services to promote health and wellness. As part of the Vital Brooklyn Initiative, an ongoing community development program for underserved neighborhoods in Central Brooklyn, Herkimer Gardens offers seniors a residential and supportive housing experience that integrates green space with urban living and recreation, creating a meditative atmosphere in an innovative facility.

 

In meeting the various needs of individual seniors and focusing on enriching their quality of life, Herkimer Gardens ensures residents have easy access to essential healthcare services, recreation, and nourishment. Residents can hang out with friends at the open space green roof or community center, learn and relax in the library/computer lounge, or workout in the new gym. Other available recreational activities offered include yoga, cooking, crafting, and tenant parties, amongst many more. In addition to housing and recreation, Herkimer Gardens will extend a large on-site urgent care facility and a food access assistance center.



The Barbara Faron Residence, named after RiseWell Community Services CEO, Barbara Faron, is composed of 118 units in a single nine-story low income building located in the heart of Far Rockaway.

A significant addition to RiseWell Community Services’ Supportive Housing Program, this state-of-the-art building includes a laundry facility, community room, exercise room, reading room with access to a terrace with great views, and 24/7 front desk and security personnel. The Barbara Faron Residence prioritizes the diverse needs of each resident, aiming to enhance their quality of life by providing convenient access to healthcare resources, recreational activities, and wellness initiatives.

In this development, community residents find more than just a safe haven; it serves as a solid foundation for growth, a cherished home, and a thriving environment where they can flourish.



The Residential Transitional Support team works with individuals to assist with developing necessary skills to obtain or maintain their independence. RTS provides intensive and enriched services for those living in Suffolk County. Visits can be made frequently with intensive support provided whenever necessary. Each plan provided by the RTS team is a person-centered service plan and based on the individual’s preferences. 

Our two teams are comprised on Masters level behavioral health workers, RNs, LPNs, direct care staff, and peer specialists, available 7 days a week and on-call 24/7.
