Outreach Services

RiseWell Community Services provides care coordination services in Queens, Kings, Suffolk and Nassau Counties. Individuals diagnosed with serious mental illness, chronic medical conditions, and/or substance abuse disorders are eligible to receive a care coordinator who will help that individual manage all aspects of their healthcare.

If you or someone you know are dealing with serious mental illness, chronic medical conditions, or substance abuse disorders, our incredible care coordination team is here to help. We connect you with behavioral health specialists, primary medical care, housing services, vocational support, and entitlements. Within 24 hours, our team will reach out and begin assistance within the first 5 business days. Our goal is to make sure the participant stays on top of appointments and receive the vital care needed for a healthy life! RiseWell offers personalized care coordination across Queens, Suffolk County, and Nassau County. 



RiseWell works effortlessly to grant people to long-term support and help them reclaim their lives. Whether on the streets, in shelters, or elsewhere, we start where the help is needed. The P.O.W.E.R Program is here for those facing homelessness and living with a mental illness or co-occurring disorders. In Suffolk County, we assist with housing, entitlements, medical coverage, and employment. 

Every Wednesday, our Patchogue Community Center offers the evening meal program, along with peer support, clothing assistance, a free hot meal, on-site food pantry, employment group, and advocacy provided regularly. A bright future starts with RiseWell!

Peers are a vital part of the recovery process at RiseWell. We believe that everyone, no matter their challenges or journey, has the capacity to recover and deserves the chance! Peers offer incredible insight, empathy, accounts of personal experience, and through trusted relationships, empower individuals to take control and rediscover hope. Hope is the key to engagement, motivation, and follow-through, and our peer support specialists not only provide hope, but serve as living proof that recovery is achievable.

Hospital-Based Peer Services

RiseWell’s Peer Support Services assist people towards recovery and offer self-help groups at inpatient psychiatric units of local hospitals. Our specialists, with personal experience managing mental health issues, substance abuse issues, and chronic health conditions, provide hope and inspiration in order to help others regain control over their life. Resources include mental health providers, health service organizations, support groups, local resources, and 12-step groups, to help participants stay out of the hospital. 

Peer Bridger Services

Our experienced peer recovery specialists partner one-on-one with individuals in the inpatient behavioral unit. Together, they illuminate a path from hospital to community living, and offer a lifeline after discharge. Our hands-on approach proves that no obstacle is too big. Our recovery specialists work intensively to ensure a seamless transition and provide inspiration for lasting recovery!

Peer Respite Services (Respite Plus)

Are you or someone you know a caretaker in need of relief? 

RiseWell’s Respite Plus Program offers breaks to caretakers working for a family member or loved one with a mental illness. Our trained respite workers will provide companionship-level care and support that fosters an independent lifestyle and an improved quality of life. Services include peer support, home visits, companionship, and community outings with an emphasis on socialization. Overnight stays are available at our respite apartment. After-Hours Social Program

RiseWell Community Services’ After-Hours Social Program operates two peer-supported, self-help community centers for adults with mental illness. Those attending have the option to join in various activities, drop in, have a meal, and perhaps meet new friends! The program is designed to offer participants an opportunity to socialize and have fun in a safe, informal, and relaxed setting. Participants can visit as often as they like at either or our two locations. Free of charge, of course.

Our Supported Employment Services are offered to those recovering from mental illness who are ready to engage in meaningful work! Whether returning to the workforce or entering it for the first time, our program is designed to focus on an individual’s skills and strengths in order to help them find and successfully maintain employment. We guide individuals to make informed decisions about their career paths with support including resume creation, job searching, job applications, prep for interviews, and developing work skills…with continued support! Through our program, participants also receive assistance in planning and managing SSI and SSDI as well as other public benefits.

RiseWell Community Services can work with individuals seeking employment through the Supported Employment Program at ACCESS-VR. This intensive program is for individuals who need support with finding, securing and successfully maintaining competitive employment in the community.  RiseWell’s Employment Specialists work one-to-one with individuals in order to assist each person with meeting his or her employment goals.  Individuals who may be eligible for this program must be highly motivated to find employment, in need of job coaching whether on the job or off-site, in need of ongoing support and must be able to get to and from the work site independently.

RiseWell Community Services is an Employment Network (EN) that provides employment support services to individuals with mental illness through the Ticket to Work and Self-Sufficiency Program.  The goal of this free and voluntary program is to assist Ticket beneficiaries in finding employment and working towards financial independence.  To be eligible to participate, the beneficiary must be between the ages of 18-64, an SSDI or SSI recipient, and have the goal of eventually becoming self-sufficient.  If the beneficiary is making timely progress in the program, the Continuing Disability Review (CDR) can be delayed and the beneficiary may also be able to maintain their health benefits.

We believe that people with common life experiences can be mutually supportive and, through shared activities, help each other to reach goals and foster increased self-worth!  RiseWell of Organization’s Companion Program provides opportunities for people in recovery to explore vocational goals by volunteering at community sites such as social programs, adult homes, hospitals, outreach programs, food pantries and soup kitchens.  Through the principles of peer support and self-help, companions strive to improve the quality of life for other individuals within the mental health community. Participants in the Companion Program may receive a meal allowance, travel reimbursement; paid vacation, sick and personal days.  They also receive on the job training and support.  In addition, individuals can benefit from involvement in social activities, peer support and friendship!

RiseWell Community Services provides a variety of financial management programs to assist individuals with disabilities.

The goal of the Financial Management Program is to increase an individual’s financial stability, maintain safe housing and develop their financial skills. RiseWell assists the participant with support in order to achieve self-sufficiency and independence!

RiseWell Community Services can serve as a Representative Payee and provide financial management services to individuals with a primary psychiatric diagnosis who are experiencing difficulty managing their finances. Individuals may voluntarily participate in the program or come to us under a court order. A doctor must certify in writing the individuals’ need for representative payee services.



RiseWell Community Services’ Veterans Fiduciary Program provides money management services to disabled service connected veterans. This is a fee for service program with the fee set by the Veterans Administration. The VA appoints RiseWell to serve as fiduciary based on the veteran’s inability to manage or direct their own finances.

As fiduciaries, we work with the veteran, their social worker and field examiner to develop a working budget. We utilize the veteran’s VA benefit to pay rent, utility costs, medical expenses and any other miscellaneous expenses. We issue a VA approved personal needs allowance based on the individual’s needs and resources.

RiseWell is responsible for reporting any changes that could affect the veteran’s benefits. These changes can include: change of address, change of income, death, incarceration and hospitalizations. These changes are reported to the VA Field Examiner and/or the VA Fiduciary Unit.

We are required to maintain a federally insured savings account(s) for the veteran. Any funds not spent on the veteran’s behalf are saved in these interest bearing accounts. RiseWell is required to maintain all financial statements with regards to these accounts. A yearly accounting is provided to the Department of Veterans Affairs and the veteran.

RiseWell Community Services Community Trust Program

Many people are concerned that an inheritance or a sudden windfall of money may cause difficulties for individuals with disabilities who are receiving government benefits.  Assets can disqualify an individual from receiving SSI and Medicaid benefits in New York State.

In response, RiseWell Community Services has established a community pooled trust for individuals with disabilities. RiseWell’s Community Trust Program will provide for the future needs of people with a physical, mental, or developmental disability without jeopardizing public benefits. Funds from the trust can be used to supplement the individual’s needs.

The Community Trust allows individuals with disabilities, their parents or grandparents to fund a trust account on behalf of the individual.

At RiseWell, we believe in wellness through purpose and connection! The Foster Grandparent Program offers volunteer opportunities to low-income seniors who wish to remain active in their communities by providing one-to-one assistance to children in elementary schools, Head Start Centers and day care centers throughout Suffolk County. Our goal of this heart-warming initiative is to make a lasting impact in a child’s life, while bringing newfound richness to the seniors involved. 

These children, identified by their teachers as needing special attention, may face learning difficulties, emotional challenges, or language barriers. Our Foster Grandparents come to the table with compassion and dedication, and serve as tutors, mentors, and emotional anchors in order to help students navigate their journey. 

Since 1977, RiseWell Community Services has proudly sponsored the Foster Grandparent Program in Suffolk County, fostering connections that span generations and create lasting memories!

Our extraordinary Senior Companion Program truly highlights the power of companionship. This program brings healthy, older individuals together with seniors facing frailty, isolation, or being homebound. It’s not just about assistance; it’s about creating fuller, more independent lives. 

Our Senior Companions go beyond practical support and provide a lifeline of friendship and connection. Companions also provide friendly visits and socialization, help with preparing light meals, medication reminders, light household duties and accompaniment on occasional outings and errands.For families, it’s peace of mind knowing their loved ones are in great, caring company.

The Suffolk County Respite Care program allows caregivers to take a well-deserved break so they can do what they need to do! We know it can be stressful and time consuming to be a caretaker- let RiseWell help! Caretakers can have peace of mind knowing that their loved one is safe and secure in the comforts of their own home.

We are dedicated to providing assistance and support to those caring for family members, friends or loved ones throughout Suffolk County. This program is funded by the Suffolk County Office for Aging and NYS Office for Aging.

Respite Care Services Provide:
