Children's Services
RiseWell Community Services provides Care Coordination services to children and youth on Long Island and in New York City. Care Coordination provides family-and-youth driven services which foster health promotion.
RiseWell’s Children’s Care Coordination Program provides services to children and youth across Long Island and NYC up to age 21, who have serious emotional disturbance, complex trauma, and/or two or more chronic health conditions. Our program is focused on promoting health and wellbeing through tailored, personalized care plans designed to help children and families achieve their goals. Whether it is immediate or long-term support, we pave the way for fuller, healthier lives and ensure that children are getting the help that they need.
We collaborate with families, health care providers, school professionals, and additional supports to provide the most comprehensive services to the child and family. This includes referrals and linkage to medical, mental health, and substance abuse providers, with continuous coordination for the child’s physical and behavioral health needs. There is no fee, services are covered through Medicaid.
Other Services Included:
- Assistance with accessing transportation, income benefits, in-surance, housing as well as any other social service needs
- Assistance with educational goals and working in conjunction with school professionals
- Linkage to vocational programs and employment opportunities
- Connections to wellness programs, afterschool activities and recreational events
- Ongoing support, monitoring, and crisis intervention

Through our Home-BEST program, we assist families that have a child in crisis by helping the youth remain in the community and avoid residential placement. Our program is personally tailored and family centered. Home-BEST is an intensive in-home program that will provide families with community resources that will help them stay connected. This impactful program focuses on strengthening the connections between youth, their families and community networks.
We provide counseling services, parenting sessions and crisis intervention as needed. Families receive education and tools in order to feel better equipped in a crisis. Our team of professionals will provide serves where children and families are most comfortable, whether that is at home or in the community.
RiseWell Community Services is working on behalf of Nassau County to provide a Home-Based Empowerment Support Team (Home-BEST).
Home-BEST Services are for children who are under 21 years of age and living in Nassau County. We provide services to those in need of critical mental health services and are at risk of hospitalization or out of home placement, regardless of insurance status.

RiseWell Community Services offers mental health and substance use services that assist children/youth and their families the power to improve their health, well-being and quality of life. These services strengthen families and help them make informed decisions about their care through evidence-based practices. Primary goals include rebuilding relationships, enhanced communication, learning coping skills, incorporating therapy into everyday practice, assessments for mental health and/or substance use needs, and more.
Services are available at no cost to those under 21 with New York State Children’s Medicaid or if enrolled in a Medicaid Managed Care Plan and have mental health and/or substance use needs.
The following services are provided at their home or in the community:
- Individual, group, or family therapy services (assessments for mental health and/or substance use needs)
- Family peer support services (provided by a Credentialed Family Peer Advocate/Certified Recovery Peer Advocate)
- Rehabilitation services (Psychosocial rehabilitation and Community Psychiatric Treatment)

* Medicaid Managed Care Plans are health insurance companies that oversee and coordinate care. Children and youth who are enrolled in a Medicaid Managed Care Plan will get the most of their Medicaid benefits through that health plan
RiseWell’s Children’s Home and Community Based Services (HCBS) is here to provide personalized and flexible care for children and youth, creating a supportive environment where they can thrive at home and in the community. We assist children and youth in achieving their goals and succeeding in various environments such as home and school. HCBS is designed as tailored, individualized plans for health, mental health, substance use treatment, and developmental support for each child/youth.
Services Include:
- Caregiver/Family support through training, education, and resources
- Community Self Advocacy Training support for understanding developmental, medical, mental health, and/or substance use needs
- Prevocational services to strengthen skill development in youths 14 or older
- Supported Employment services for youths 14 or older with ongoing assistance in job finding, coaching, and benefits as well as workplace support

RiseWell Community Services is offering outpatient addiction services for adults and children/adolescents diagnosed with a substance use disorder and for individuals affected by a family member’s substance use. These services are designed for individuals seeking support to maintain sobriety. Individuals will learn the effects substance use has on the body, mind and on interpersonal relationships. Individuals will also learn how to break the cycle of physical and psychological addiction. The program is licensed by the New York State Office of Addiction Services and Support (OASAS).
