We understand that the needs of our seniors vary, from intensive clinical and behavioral treatment to aging in place to volunteer opportunities and mentorship. We aim to provide seniors across New York the essential services required to thrive!

Whether it is providing outreach and treatment with our mobile disciplinary team, welcoming those in need to a hot meal, or providing recreational activities to increase socialization, we have multiple services that provide day-to-day support for those in recovery.

Through our transformative supportive housing services, RiseWell empowers individuals to rebuild their lives and thrive within a caring community. We know that having a home can be crucial to health. Through direct support, we encourage recovery and growth, with many programs providing residents with skills to move on to more independent housing. Some of our residential services include Affordable Mixed Use Housing, Supportive Scattered-Site Housing, Intensive Support Apartment Programs, and Community Residences.
Services
RiseWell Community Services is now offering outpatient addiction services for adults and children/adolescents. Services are designed to help each person gain support and maintain sobriety. Individuals will learn the effects substance use has on the body, mind and interpersonal relationships and will learn how to break the cycle of physical and psychological addiction. Family members involved will gain an understanding of addiction and develop skills to support their loved ones and take care of themselves.

From hospital-referred intensive care to independent therapy and beyond, RiseWell tailors mental health services to an individual’s needs. We offer a wide array of services through our intensive outpatient programs including individual and group therapy, medication management, substance abuse recovery services, health screening, remote telehealth as well as in-home treatment. RiseWell Community Services Clinical Services Department serves over 1,400 individuals at a time through Personalized Recovery Oriented Services (PROS) Programs, Outpatient Mental Health and Substance Use Clinics, and Assertive Community Treatment (ACT) Programs.

RiseWell Community Services offers over 800 permanent, safe and affordable residential placements in our Supportive Scattered-Site Housing Program. In this program, individuals with disabilities live in apartments or houses throughout Nassau, Suffolk, Queens, Brooklyn and the Bronx.

RiseWell Community Services provides care coordination services in Queens, Kings, Suffolk and Nassau Counties. Individuals diagnosed with serious mental illness, chronic medical conditions, and/or substance abuse disorders are eligible to receive a care coordinator who will help that individual manage all aspects of their healthcare.

RiseWell Community Services provides Care Coordination services to children and youth on Long Island and in New York City. Care Coordination provides family-and-youth driven services which foster health promotion.
