Substance Use and Addiction Services
RiseWell Community Services is now offering outpatient addiction services for adults and children/adolescents. Services are designed to help each person gain support and maintain sobriety. Individuals will learn the effects substance use has on the body, mind and interpersonal relationships and will learn how to break the cycle of physical and psychological addiction. Family members involved will gain an understanding of addiction and develop skills to support their loved ones and take care of themselves.
RiseWell Community Services is offering outpatient addiction services for adults and children/adolescents diagnosed with a substance use disorder and for individuals affected by a family member’s substance use. These services are designed for individuals seeking support to maintain sobriety. Individuals will learn the effects substance use has on the body, mind and on interpersonal relationships. Individuals will also learn how to break the cycle of physical and psychological addiction. The program is licensed by the New York State Office of Addiction Services and Support (OASAS).
RiseWell Community Services provides a mobile substance abuse program utilizing clinical treatment to serve members of the LGBTQIA+ community. Our mobile team will provide services anywhere in the community, including your home. We provide tailored, comprehensive substance abuse and mental health assessment and treatment to those struggling with addiction. These services are free of charge.
Services Include:
- Individual therapy
- Peer support
- Psychiatric evaluation
- Medication management
- MAT (Suboxone- used to treat opioid addiction)
- Narcan training (for overdose prevention)
- Free access to safer use supplies

RiseWell Community Services’ Mobile Opioid Support Team offers free opioid use support, education, and access to harm reduction tools. Complete with treatment rooms, our vehicle travels throughout Suffolk County to provide opioid recovery support services to adults 18 years or older (or under 18 with parental consent) with opioid use disorder.
Individuals utilizing our services have access to our behavioral health services and network of trained professionals. Our Mobile Opioid Support Team is here to help and provide integrated treatment and support that’s safe, convenient, confidential, and tailored to the individual’s needs.
Services are free of charge. Insurance and appointments are not required.

- Medicated-assisted treatment (Suboxone)
- Overdose prevention/ Narcan training
- Peer support
