Day-to-Day Support

Whether it is providing outreach and treatment with our mobile disciplinary team, welcoming those in need to a hot meal, or providing recreational activities to increase socialization, we have multiple services that provide day-to-day support for those in recovery.

RiseWell Community Services’ After-Hours Social Program operates two peer-supported, self-help community centers for adults with mental illness. Those attending have the option to join in various activities, drop in, have a meal, and perhaps meet new friends! The program is designed to offer participants an opportunity to socialize and have fun in a safe, informal, and relaxed setting. Participants can visit as often as they like at either or our two locations. Free of charge, of course.

RiseWell’s Children’s Home and Community Based Services (HCBS) is here to provide personalized and flexible care for children and youth, creating a supportive environment where they can thrive at home and in the community. We assist children and youth in achieving their goals and succeeding in various environments such as home and school. HCBS is designed as tailored, individualized plans for health, mental health, substance use treatment, and developmental support for each child/youth. 

Services Include: 

  • Caregiver/Family support through training, education, and resources
  • Community Self Advocacy Training support for understanding developmental, medical, mental health, and/or substance use needs
  • Prevocational services to strengthen skill development in youths 14 or older
  • Supported Employment services for youths 14 or older with ongoing assistance in job finding, coaching, and benefits as well as workplace support 

Children/Youth and their families/caregivers can speak with a Health Home care manager, or call the Children and Youth Evaluation Service at 631-321-8229 Ext. 1229 for information and referral

Provides empowerment services, peer support, psychosocial rehabilitation, education and employment support, family support and training, as well as community psychiatric treatment.

We believe that people with common life experiences can be mutually supportive and, through shared activities, help each other to reach goals and foster increased self-worth!  RiseWell of Organization’s Companion Program provides opportunities for people in recovery to explore vocational goals by volunteering at community sites such as social programs, adult homes, hospitals, outreach programs, food pantries and soup kitchens.  Through the principles of peer support and self-help, companions strive to improve the quality of life for other individuals within the mental health community. Participants in the Companion Program may receive a meal allowance, travel reimbursement; paid vacation, sick and personal days.  They also receive on the job training and support.  In addition, individuals can benefit from involvement in social activities, peer support and friendship!

Our extraordinary Senior Companion Program truly highlights the power of companionship. This program brings healthy, older individuals together with seniors facing frailty, isolation, or being homebound. It’s not just about assistance; it’s about creating fuller, more independent lives. 

Our Senior Companions go beyond practical support and provide a lifeline of friendship and connection. Companions also provide friendly visits and socialization, help with preparing light meals, medication reminders, light household duties and accompaniment on occasional outings and errands.For families, it’s peace of mind knowing their loved ones are in great, caring company.

Eligibility Requirements

Applicants must be aged 55+, income eligible to receive stipend, in good health, able to pass a background check, motivated, enthusiastic, have a desire to help others. No background in health care needed.
