Care Coordination

At RiseWell, we believe that the first step to good health is ensuring that individuals and families have a place to live, access to basic needs such as food, clothing, and transportation, and have a strong support system. Our Care Coordination programs serve both adults and children to support wellness and improved quality of life.

If you or someone you know are dealing with serious mental illness, chronic medical conditions, or substance abuse disorders, our incredible care coordination team is here to help. We connect you with behavioral health specialists, primary medical care, housing services, vocational support, and entitlements. Within 24 hours, our team will reach out and begin assistance within the first 5 business days. Our goal is to make sure the participant stays on top of appointments and receive the vital care needed for a healthy life! RiseWell offers personalized care coordination across Queens, Suffolk County, and Nassau County. 



RiseWell’s Children’s Care Coordination Program provides services to children and youth across Long Island and NYC up to age 21, who have serious emotional disturbance, complex trauma, and/or two or more chronic health conditions. Our program is focused on promoting health and wellbeing through tailored, personalized care plans designed to help children and families achieve their goals. Whether it is immediate or long-term support, we pave the way for fuller, healthier lives and ensure that children are getting the help that they need. 

We collaborate with families, health care providers, school professionals, and additional supports to provide the most comprehensive services to the child and family. This includes referrals and linkage to medical, mental health, and substance abuse providers, with continuous coordination for the child’s physical and behavioral health needs. There is no fee, services are covered through Medicaid. 

Other Services Included: 

  • Assistance with accessing transportation, income benefits, in-surance, housing as well as any other social service needs
  • Assistance with educational goals and working in conjunction with school professionals
  • Linkage to vocational programs and employment opportunities
  • Connections to wellness programs, afterschool activities and recreational events
  • Ongoing support, monitoring, and crisis intervention
