Our mobile teams bring essential services to individuals and the community. From substance use and overdose prevention services to intensive support services, our teams ensure that all have access to quality care

RiseWell Community Services provides a mobile substance abuse program utilizing clinical treatment to serve members of the LGBTQIA+ community. Our mobile team will provide services anywhere in the community, including your home. We provide tailored, comprehensive substance abuse and mental health assessment and treatment to those struggling with addiction. These services are free of charge. 

Services Include: 

  • Individual therapy 
  • Peer support
  • Psychiatric evaluation
  • Medication management
  • MAT (Suboxone- used to treat opioid addiction)
  • Narcan training (for overdose prevention)
  • Free access to safer use supplies

The Residential Transitional Support team works with individuals to assist with developing necessary skills to obtain or maintain their independence. RTS provides intensive and enriched services for those living in Suffolk County. Visits can be made frequently with intensive support provided whenever necessary. Each plan provided by the RTS team is a person-centered service plan and based on the individual’s preferences. 

Our two teams are comprised on Masters level behavioral health workers, RNs, LPNs, direct care staff, and peer specialists, available 7 days a week and on-call 24/7. 

  • Medication management and education
  • Community reintegration (including travel training)
  • Daily living skill development
  • Assistance with medical and behavioral health appointments
  • Coordination with participant’s treatment and support team
  • Budgeting and entitlements training
  • Counseling services
  • Substance abuse services and support
  • Crisis intervention (including access to 24 hour emergency line and after-hours warm line)

To Apply: Fill out the RTS Referral Form below and send information listed on the form. Please include a copy of the most recent psychological, psychiatric summary and physical. 




The goal of Mobile Residential Support is to ensure that that participants can maintain their housing, further develop community and daily living skills, and increase their independence. The MRS team provides intensive support services to individuals who reside in Nassau County Scattered Site Supportive Housing within the SPA Housing system.

 Services include:

  • Medication management and education
  • Community reintegration (including travel training)
  • Daily living skill development
  • Assistance with medical and behavioral health appointments
  • Coordination with participant’s treatment and support team
  • Budgeting and entitlements training
  • Counseling services
  • Substance abuse services and support
  • Crisis intervention (including access to 24 hour emergency line and after-hours warm line)
